“...provide and support an efficient transition program for the students
to their school, their family and their community...”
At Centre Jules-Léger, an efficient transition
program would include the following steps:
- An information session relative to
their
child’s
upcoming
return
to the community and school is presented to
the
parents
through one of the School Council’s
regular
meeting.
- A transition document is compiled for
the
benefit
of the returning student.
The
document
would
include a copy of the student’s current IEP and would
highlight his
strenghts and needs. Information required for IAS’s would be
included in this
document.
- A meeting is scheduled to allow student,
parents,
school
board
officials
and staff
to
share the information required for
an
efficient
and
seamless transition of the student to his
or her host school.
- Subsequent coaching is provided by
staff,
social
worker,
speech
pathologist and
psychologist
to both parents and school board
officials.
- Whenever possible and desirable, Centre
Jules-Léger
facilitates the
partial insertion of a student within
the host school
before he is finally released at the end
of his program with us.
- Post-tests results are shared with
parents
and
school
boards’ officials at the end
of the school year to show improvments
in the learning capacities of the students
and
areas which still require support.
- Verification on the quality of the
reinsertion
would
be
established
the
following
school year by staff at Centre Jules-Léger, and some support
would be provided if it was required.
